I’m Elita Chan, a credentialed Home Medicine Review (HMR) pharmacist, with over 21 years of experience conducting in-home medication reviews. I also work as a clinical pharmacist at QEII Jubilee hospital in the Geriatric ED initiative multidisciplinary team.  I enjoy talking to people about their medications to improve their understanding and management of their medications safely and effectively.

I am deeply committed to patient education of medications, believing that informed patients are more likely to have improved health outcomes. I aim to help optimise your treatment regimens, reduce potential medication risks, and enhance therapeutic outcomes.

I believe medications are a cornerstone of patient-centred care and should be used to improve health outcomes and overall quality of life. I believe every question about medicines is valued and that no query is too small. I’m always ready to provide clear, compassionate guidance to help patients feel confident in their medication management.

I enjoy meeting new people and making new friends.  I love travelling with my family, eating out, crafting leather handbags and especially love playing with my furbaby Lucky the Pom-poodle.

Managing multiple medications can be complex—and ensuring they’re safe and effective is essential. A Home Medicines Review (HMR) is a valuable medicare funded service where a credentialed pharmacist visits you at home to review all the medicines you’re currently taking.

If you care for someone else, managing their medications properly is just as important. Incorrect use or combining the wrong medicines can lead to serious health issues.

An HMR is different from a medication review conducted by your doctor or in hospital. While your GP ensures you’re prescribed the right treatments for your condition, an HMR focuses on how those medicines are being used day-to-day at home.

Your GP will assess whether an HMR is appropriate for you. If so, they’ll provide a referral to a qualified pharmacist who will arrange a convenient time to visit.
